Procedures for Requesting a Transcript
1. A request for a transcript from a closed private school should be directed to
Private School Licensing Division, Department of Postsecondary Education 401 Adams
Avenue Montgomery, Alabama 36104
2. Download and complete the
Request for Transcript
Form (PDF), which asks for the following information.
- Name during attendance at the school
- Social Security Number
- Name of the school
- Where the transcript should be mailed
3. The Fee is $10.00 per transcript search.
- The fee must be in the form of a Cashier's Check or a Money Order made out to "Department
of Postsecondary Education"
